Advance Care Planning in Canada:

The Canadian Hospice Palliative Care Association (CHPCA) has run the Advance Care Planning (ACP) in Canada initiative since 2008, developing a National Framework for advance care planning in collaboration with a number of sectors and professional groups.  The initiative has created a website, which includes news on activities underway across Canada as well as a repository of tools developed for professionals and patients/individuals to assist them in making appropriate decisions regarding their care. Resources, information, and best practices are promoted through our monthly e-blasts to our members (~1200), social media accounts (Twitter, YouTube and Facebook), and our partnerships with national, provincial, and community organizations across Canada.

In 2019, the national ‘Speak Up’ Advance Care Planning (ACP) in Canada initiative has received $1.9 million over three years for a project to help people living in Canada prepare for their future health care needs. The project will involve a series of public education and awareness campaigns, support of community-based ACP programs, and promotion of ACP resources and guides. Some of the current initiative activities: 

  • Advance Care Planning National Framework 2019
    The SpeakUp initiative held upstream consultations, spring 2019, to update the National Advance Care Planning (ACP) in Canada Framework. This new Framework includes an updated plan for implementation of ACP in Canada and focuses more on broadening partnerships, and promoting further collaboration among different jurisdictions (local, provincial/territorial, national) and systems (health, law, social services, life planning).   • National Poll 
    ACP in Canada has commissioned a national poll in collaboration with Nanos (2019).  The poll asked (n= 2,948) Canadian adults about their end of life care planning; what makes it easier to have a discussion? What makes it tough to make a plan? Results are available by jurisdiction. Download the national poll infographic
  • The ‘Speak Up’ initiative website update (www.advancecareplanning.ca) 
    Full of information, tools, and tips to help families and healthcare professionals with starting these conversations.
  • New Resources 
    ACP in Canada is developing more resources in the following sectors to help Canadians prepare for their future health care:
    • Legal – Learn more about the living well planning well toolkit for lawyers and the public facing toolkit for families and patients to learn about their rights. 
    •  Long term care: upcoming 2020 
    •  Community care: upcoming 2020
  • Public Awareness 
    ACP in Canada runs an annual ACP Day campaign (April 16th) and is also developing additional campaigns to reach specific target groups and expand ACP outreach. Learn more about ACP anywhere campaign
  • Community-based ACP Programs 
    ACP initiative provides support to several community based programs in Canada.
