Membership
Membership with the Canadian Hospice Palliative Care Association (CHPCA) is available to anyone who would like to join. Membership is categorized into two main groups: Canadian and International.
Residents of Canada
For residents of Canada, membership classifications and fee structure for the CHPCA are dependent upon your province or territory of residence. Membership with the CHPCA automatically includes membership with your corresponding Provincial Association, except where noted. For further information or to become a member of the CHPCA, please follow the instructions below.

International Residents
Membership with the CHPCA is also open to persons living outside of Canada. This membership can be classified as a group/program or individual. To become an International Member of the CHPCA, please follow the instructions below.
Instructions
1) Print out the Membership Form. 
2) Choose a membership category where applicable.
3) Fill in the appropriate information.
4) Mail this form with your cheque in Canadian dollars only, to the CHPCA National Office at:
