CHPCA AWARD OF EXCELLENCE


The CHPCA Award of Excellence recognizes outstanding personal achievement at a national level in the field of hospice palliative care in Canada and promotes awareness of hospice palliative care in Canada.

PAST RECIPIENTS

Who is eligible?

Any individual who exemplifies excellence in hospice palliative care at a national level in Canada, in either a paid position or as a volunteer. Through his or her achievement, their national contribution has enhanced the quality of life for patients and families facing life-threatening illness and/or chronic progressive disease or has enhanced the profile of hospice/palliative care in all of Canada. The award will be given to only one individual each year and need not be awarded if no suitable candidate is proposed.

Nominations for the 2007 award are now closed.

The President of the CHPCA Board of Directors will notify the recipient, who will be invited to attend a presentation ceremony in conjunction with the Annual General Meeting.


How to submit a nomination?

1. Print out and complete the nomination form.

2. Attach two letters of support which clearly demonstrate the nominee’s skills, talents and accomplishments related to hospice/palliative care.

3. Include the written consent of the candidate, and mail to CHPCA, Annex B, Saint-Vincent Hospital, 60 Cambridge Street North, Ottawa, ON  K1R 7A5, or fax to (613) 241-3986.
