What are your admission/referral criteria?

Patients must be terminally ill or have a life threatening ilness with pain and symptom management issues, and posses a do not recusitate order. The patient and family must also agree with palliative care philosophies.

How would you answer the following question if asked by a patient, client, resident and/or family?

"What can you do for me?"

We address symptoms to make you comfortable during the journey and at end of life. We provide support amd comfort to you and your family during the journey, at end of life, and after passing. We can also address questions and concerns, and provide a presence for you and your family.
