What are your admission/referral criteria?

Patients facing a life threatening illness such as Cancer, CDPD, End Stage CHF, degenerative neuromuscular disease etc and who have symptom control issues.

How would you answer the following question if asked by a patient, client, resident, and/or family?

"What can you do for me?"

We assist families with the difficulties involved when an individual faces a life threatening illness. We work as an interdisciplinary team in conjunction with your primary care providers to assist with any/all issues pertaining to physical/emotional/spiritual well being of patients and  their family and loved ones.

The Nigara West Palliative Care Team (NWPCT) is a unique community team that is a results of a partnership between West Lincoln Memorial Hospital and the Hamilton Access Centre. The NWPCT provides care to patients and families in every setting care including home, McNally House Hospice, WLMH, Retirement homes, and long term Care Facilities. The NWPCT is an interdisciplinary team that works in close partnership with family physician and other local Palliative Care Services provides.
