What are your admission/referral criteria?

Community Program:  Anyone living with the challenges of life-limiting illness from the time of diagnosis can request support.

Residential Program:  Requires referral from a health care provider.  The individual needs to have a life-limiting illness with a prognosis of less than three months to live and is in need of residential hospice care.  The individual must also have agreed to allow a natural death to occur.

For more than 20 years, The Dorothy Ley Hospice has provided compassionate care for people living with the challenges of life-limiting illness or loss. Our services meet their physical, emotional, spiritual and practical needs as well as the needs of their caregivers and family members, enabling them to live life to the fullest.  Our services are focused on enhancing quality of life.  The care is individual centred and is driven by the express needs and goals of the individual.

Our community program is open to anyone living with the challenges of life-limiting illness and is available from the time of diagnosis.

Our residential program focuses on providing end-of-life care to individuals living with any life-limiting illness who have a prognosis of less than three months to live and require residential care.

All of our services are available at no cost to the individual thanks to the generosity of our donors and government funding from the Mississauga Halton Local Health Integrated Network and the Mississauga Halton Community Care Access Centre.
