We provide inpatient and outpatient consultations and ongoing support for dying  at Mount Sinai Hospital. We can provide physician-based home palliative care with 24/7 coverage for patients in our catchment area with a goal to helping patients to stay at home as long as possible and perhaps to die at home. We provide support for families, control of physical symptoms, navigation through the system and care planning coordinating with community home care resources. We also provide home pediatric palliative care for dying children. For grieving children and youth, we have grief and bereavement counselling- individual,family and group.
