South Shore Area Palliative Care Services aims to enhance the quality of life of people with a life-limiting illness, and their families/caregivers.

What are your admission/referral criteria?:

The individual has been assessed as requiring Palliative Care Services by the Intake Triage personnel.
The individual is aware of their diagnosis, agrees to the referral and is willing to accept Palliative Care Services.
The Primary Provider (attending family physician or nurse practitioner) is willing to work with the Palliative Care Consultation Team in the care of their patient. For those individuals without a primary provider, provisions for care are determined at time of admission to the Palliative Care Service.
The individual's environment is safe and suitable for the provision of Palliative Care Services.

Your hospice palliative care program/service provides support and/or care for the following:

Cardiovascular Disease
Infectious Diseases (ie HIV/AIDS, Hepatitis-C)
Neurological Disorders
Renal Disease
Respiratory Disease
Care for all conditions
Anticipatory Grief and Bereavement
Emotional/Spiritual Support

The palliative care team can assist with uncontrolled pain and symptoms, spiritual/emotional/social distress, planning for end of life, and grief. Our volunteers can provide companionship for patients and families/caregivers.
