What are your admission/referral criteria?

Terminal illness, 6 months or less, or clients with a longer tem illness requiring support and resources to prepare for end of life.

How would you answer the following question if asked by a patient, client, resident and/or family?


“What can you do for me?”


We can provide access to palliative nurse who will do home visits and clinical assessment.  The team has a palliative physician who is available to help with pain and symptom management.  The nurse can make referrals to the volunteer coordinator for your community.  Also the nurse can assist you with accessing resources that you need.

Through the provincial program you can access the provincial drug access program if you have been given a diagnosis of 6 months or less.  Should you wish to die at home the palliative nurse in consultation with the home care program will put in the appropriate personal to help your family.  This may be a nurse and or a health care aide.
