The IH West Hospice Palliative program mission is to ensure the provision of high quality, integrated hospice palliative services to people and their families who are living with a life threatening illness throughout both the duration of the illness and bereavement.

What are your admission/referral criteria?

  • Palliative performance Score 60%
  • The referral must be aware they have a life-threatening illness
  • Accept the support of the hospice palliative program

How would you answer the following question if asked by a patient, client, resident and/or family?

“What can you do for me?”

We have a team of supports that will be there to answer your questions, offer support and provide hands on care, if requested, to alleviate the feeling of being a burden to your loved ones.

We can provide the information, support you have identified as needing in order to have quality of life until you die.

We guide you and your family on the journey through the end stage of disease, provide you and your family with resources to allow you to preplan your next steps and to provide and teach you how to manage your symptoms.
