What are your admission/referral criteria?

Adults only for both our programs. Day hospice clients must be ambulatory, cognitive and able to take own medications.

How would you answer the following question if asked by a patient, client, resident, and/or family?

"What can you do for me?"

We can offer support by way of education and knowledge of end-of-life issues and hospice. We can help with the grieving over a loved one. We can invite any adult living with a life-limiting illness, a respite and enjoyment for a few hours, twice a month in our day hospice program.
