To empower family based caregivers to provide patients with quality palliative care through timely access to information, programs of support and services.

What are your admission/referral criteria?

Any individual or family member coping with a terminal illness may be referred to the program.
Referrals can be made by health care professionals, family members or individuals.

How would you answer the following question if asked by a patient, client, resident and/or family?

“What can you do for me?”  

We can provide in-home support through a variety of ways.  We provide individuals and family members with emotional support in coping with terminal illness. We also have many trained, screened volunteers that go into the home to do a variety of tasks including respite care - sitting with a client to allow the family member or caregiver a break, running errands, reading to a client, doing light housekeeping duties, etc. In addition, we also offer education to individuals in the community.
