What are your admission/referral criteria?:We perform a free assessment to determine the level of care required. Typically we require a minimum of 4 hours of care per week.

Your hospice palliative care program/service provides support and/or care for the following:
Cardiovascular Disease
Infectious Diseases (ie HIV/AIDS, Hepatitis-C)
Neurological Disorders
Renal Disease
Respiratory Disease
Care for all conditions
Anticipatory Grief and Bereavement
Emotional/Spiritual Support

When dealing with the reality of the palliative stage, being surrounded by the familiar environment of home can in itself provide some comfort and relief. At Home Care Assistance, we can help make the last days of life more comfortable and enjoyable within our clients own homes.
