Evergreen nursing services offers immediate, personalized care, flexible to the changing medical needs of the client. Evergreen is owned and operated by a Canadian Registered Nurse and is dedicated to providing the highest quality personalized professional health services.

  • Life threatening illness
  • Live in Markhamor Whitchurch-Stouffville

How would you answer the following question if asked by a patient, client, resident and/or family?

“What can you do for me?”

  • Psycho-social support – discussion about illness and how it impacts the client and family
  • Assistance with system navigation if not already a client of CCAC
  • Respite for Family/Friend Caregiver
  • Caregiver support group
  • Practical support –Assistance with activities at home that are not medical or nursing
  • Loan of small equipment
  • Accompaniment to medical appointments locally
  • Anticipatory grief support for client and family including children
  • Evergreen Living Room drop-in program with lunch included
  • Bereavement support for all ages individual and group formats
