What are your admission/referral criteria?

  • Women carrying fetuses with a lethal or potentially lethal congenital anomaly who may or may not survive birth.
  • Infants, children and adolescents with progressive, life-limiting disease.
  • Infants, children and adolescents with life-limiting diseases whose complications of disease are leading to an overall decline in health and wellbeing.
  • Paediatric palliative pain and symptom management.
  • End of life care.
  • Bereavement care.

 Improve quality of life of life by:

1.  pain and symptom management
2.  addressing the pychological, emotional and spiritual issues of living and dying for child and family
3.  engaging in advance care planning and co-creating advanced directives
4.  support with complex decision making
5.  providing continuity of care from hospital to community
6.  explore ways to create memories and leave a legacy
