What are your admission/referral criteria?

Admisson to Hospice depends on many factors including if the client lives alone, family support, how well they are being managed at home, how well the family is coping, how much emotional, psychosocial needs are in place, how well the client is coping and what their needs and requests are.

Referral criteria includes a PPS of less than or equal to 40%.

How would you answer the following question if asked by a patient, client, resident and/or family?

“What can you do for me?”

Hospice palliative care supports people living with life-threatening illnesses.  It provides care for the needs of patients and their loved ones, whether medical, psychological, social, spiritual, or practical.  Physicans, nurses, physio/occupational therapists and volunteers work together with friends and family to ease a difficult life passage.  The Hospice is home when home is no longer possible or practical, making the last days of a person’s life together with their family peaceful, comfortable and meaningful.
