  • Full awareness of diagnosis and prognosis and acceptance of hospice care
  • Valid OHIP
  • Eligibility for CCAC services
  • Consents for DNR, hospice admission, and POA's

The hospice will be a full partner in your care; minimize your pain; will have your decisions and choices respected and followed, will be treated with openness and honesty, without deceptions or half-truths, will receive quality medical and nursing care who have compassion, sensitivity and knowledge.
