Dartmouth General Hospital Palliative Care Interdisciplinary Team provides physical, spiritual and psychosocial Support to patients and their families. Organizing care in hospital and assisting with discharge planning and Bereavement follow-up. Working closely with the Capital Health Integrated Palliative Care Service: Home Team, Clinic, Continuing Care & VON.

We provide support for pain and symptom management as well as discharge planning and follow up.

Additional Information

Dartmouth General no longer has it's own Palliative Care Program.  It is now under the Capital Health Palliative Care Program which supports Dartmouth General with a Full Time Consult Nurse (Angela McFadden) and Physician support two 1/2 days a week.  Angela McFadden works with the inhouse Interdisciplinary Team but they are not specifically trained for Palliative Care.
