What are your admission/referral criteria?

We are a home care nursing agency and typically receive our referrals through CCAC. Admission and referrals are based on client needs. 

How would you answer the following question if asked by a patient, client, resident and/or family?

“What can you do for me?”  

Offer staff who can provide nursing care and support at the end of life, provide respite care so that family can  rested and able to support you. Provide medication management, connect with family Dr and other service providers as required.
