What are your admission/referral criteria?

We are a home care nursing agency and typically receive our referrals through CCAC.  We are also receive private inquiries from the families of palliative clients who are looking to complement the care they may already be receiving from CCAC.

How would you answer the following question if asked by a patient, client, resident and/or family?

"What can you do for me?"

Bayshore is able to provide families/clients with a team of experienced caregivers/registered staff who will provide excellent client care and respite for family members.  Our caregivers are available to service clients 24 hours a day, 7 days a week.  Bayshore will liaise with other agencies and assist families with all of their health care related questions including questions about funding options, equipment/supplies in the home etc. 

We promise our clients that we will....

  • Have a person available to answer their call 24/7.
  • Listen to them and take the time to understand their needs.
  • Explain the full range of services available and help them navigate the home care system.
  • Provide a no-charge needs consultation at the place most convenient to them.
  • Provide professional caregivers who are licensed or certified in their field.
  • Have a nurse conduct an assessment of their care needs prior to starting their care.
  • Provide them with an individualized care plan that will be reviewed regularly by a nurse care manager.
  • Ensure our caregiver matches thir needs, and provide a replacement if they do not.
  • Strive to provide the same primary and back-up caregivers.
  • Work with other health care professionals to meet their care needs.
