This service offers supportive palliative care to terminally ill patients and their families.   The team is comprised of palliative care physicians, clinical nurse specialists, social workers, spiritual care provider and other allied health care professionals (composition varies depending on location).

Services provided may include:

  • management of symptoms (e.g.  pain, breathing difficulties, nausea etc..)
  • maintenance of optimal physical and cognitive function
  • psychological and spiritual support
  • patient/family education regarding disease progression and the end of life
  • prognosis and goals of care
  • discharge planning and coordination of palliative care services
  • transitioning to hospice
  • grief counseling

Locations offered:

  • Foothills Medical Center (FMC), Rockyview General Hospital (RGH), and Peter Lougheed Center ( PLC)
  • Integrated Home Care – including Palliative Home Care
  • Long term care & Supported living
  • Rural area including hospitals and home care
  • Selected outpatient clinics
  • Hospices:

How to consult:

  • FMC, RGH, PLC: Ask your attending doctor or nursing staff to initiate a palliative care consult
  • Homecare and Palliative home care:  Ask your family doctor to fill out a referral to general home care; OR call 403.943.1920 to initiate a self referral to general home care.  Then your general home care team can initiate a referral to palliative home care.
  • Long term care & Supported living:  Ask your attending doctor or nursing staff to initiate a palliative care consult
  • Rural area including hospitals and home care:  Ask your family doctor or attending doctor or nursing staff to initiate a referral to the rural palliative care consult team
  • Hospices:  Ask your attending doctor or nursing staff to initiate a palliative care consult
