The Canadian Hospice Palliative Care Association (CHPCA) is pleased to provide the resources listed below to promote National Hospice Palliative Care Week. This year marks the third year of a three-year campaign to raise awareness of the importance of Advance Care Planning. Advance Care Planning is a process by which patients and health care professional(s) discuss an individual’s wishes about their health care choices at the end-of-life. This year’s theme is “Advance Care Planning – Let’s Talk about It”.
