National
Hospice Palliative Care Week is a time to raise awareness, celebrate,
recognize, and share the achievements of hospice palliative care in your
community and throughout the nation. National
Hospice Palliative Care Week provides hospice palliative care programs
and services across This year
marks the second year of a three year campaign to raise awareness of the
importance of Advance Care Planning. Advance
Care Planning is a process by which you and your health care
professional(s) discuss your wishes about what health care choices you want
at the end-of-life. The focus of this year’s campaign
is the role of Primary Health Care Providers in end-of-life care. CHPCA will
be distributing national posters and other resources in the next two
weeks. In addition we will be updating our website with electronic
resources created for the Week by the end of March. If your program or service is registered with the CHPCA’s On-Line Directory of Hospice Palliative Care Programs and Services you will automatically receive this year’s resource kit in the mail.
