Become a CHPCA Associate or Affiliate / Provincial Member
CHPCA Associate status with the Canadian Hospice Palliative Care Association (CHPCA) is available to anyone who would like to join. Associates (Individuals) or Affiliates (Groups) are categorized into two main categories: Canadian and International.
Residents of Canada
For residents of Canada, classifications and fee structure for the CHPCA are dependent upon your province or territory of residence. Becoming an Associate or an Affiliate with the CHPCA automatically includes membership with your corresponding Provincial Association, except where noted. Benefits of Becoming a CHPCA Associate or Affiliate and a Provincial Member
Joint associate or affiliate status & provincial membership significantly enhances the overall legitimacy of the hospice palliative care movement in Canada.
Credibility in numbers:
The larger the associate or affiliate status & provincial membership base the more effective Associations are in many aspects of their mandates, i.e. awareness raising, fundraising, program development and national standards.
Joint associate or affiliate status & provincial membership allows for the pooling of resources to further the overall mandate and support the development and implementation of programs/services across the country. Pooling of resources also minimizes duplication.
Joint associate or affiliate status & provincial membership helps ensure that knowledge, programs, services and expertise are portable across the country.
Joint associate or affiliate status & provincial membership provides access to information, expertise and colleagues all across the country.
Associate and affiliate status with the CHPCA is also open to persons living outside of Canada. This status can be classified as a group/program or individual. Individuals are called Associates and Group/Programs are called Affiliates.

- Check the boxes that apply to your type of application
- Pay by credit card via PayPal (you do not need a PayPal account) or Mail this form with your cheque in Canadian dollars only to the CHPCA National Office at:

The Canadian Hospice Palliative Care Association has encouraged the establishment of 8 Interest Groups for the use of its members. These Interest Groups have been created by the members, as a means of information sharing across Canada and abroad. The members of the Interest Groups typically meet face to face each year just before the Association's Annual General Meeting.
Vehicles for sharing information include:
- Free space in the Association's newsletter AVISO
- Space on our website devoted to each of the 8 Interest Groups
Those Interest Groups interested in using any of these services are encouraged to contact the Canadian Hospice Palliative Care Association.

CHPC NG Membership (Nurses Group)
Joining the CHPC NG requires an additional step. To become a member you much fill out the CHPC NG Membership form here. CHPC NG membership run from April 1 to March 31 annually. All CHPC NG members MUST be CHPCA Associates or Affiliates at the time of renewing or signing up to become a CHPC NG member. For more information, please vist the CHPC NG webpage here.
